The PACIO Project, convened by MITRE on behalf of CMS, is a collaborative effort between industry, government and other stakeholders, with the goal of establishing a framework for the development of a Fast Healthcare Interoperability Resource (FHIR) implementation guide(s) and reference implementations to facilitate health information exchange between post-acute care and acute care facilities. The project addresses the major gaps in health information exchange during transitions of care, which is often fragmented and can lead to poor health outcomes, increased burden, and increased costs. Interoperable health information exchange has the potential to improve patient and provider communications and supports access to longitudinal health information that enables improved efficiencies, improved quality of care, and improved health outcomes. Data should be usable across the continuum of care, and beyond the traditional healthcare system – into the community.
